By submitting this application, the vendor certifies that all information provided is accurate and complete to the best of their knowledge. The vendor agrees to comply with all applicable federal, state, and local laws, as well as the policies, procedures, and safety standards of Stackwall.

The vendor further acknowledges and agrees to provide, upon request:

• A valid Certificate of Insurance (COI), naming Stackwall as a certificate holder or additional insured;
• Proof of Workers' Compensation coverage or exemption;
• Any applicable licenses, certifications, or permits required for the services or products provided.

Submission of this application does not guarantee approval or any contractual relationship. Stackwall reserves the right to approve, reject, or remove any vendor at its sole discretion.
